Pelvic organ prolapse repair using robotic-assisted sacral hysterocolpopexy vs vaginal surgery with the Uphold™ system: 1-year clinical outcomes

The study compares the clinical outcomes of robotic-assisted sacral hysterocolpopexy (RASC) and vaginal surgery using the Uphold™ Vaginal Support System for apical pelvic organ prolapse repair. Conducted as a nonrandomized, prospective, multicenter study, it included 72 women undergoing RASC and 73 receiving Uphold™ surgery. After one year, both procedures achieved high rates of anatomical success (96.4% for RASC and 93.3% for Uphold™), but reoperation for prolapse recurrence was significantly more common in the RASC group (15.3% vs. 2.8%). Although both methods significantly improved pelvic floor distress and pain, the Uphold™ procedure showed greater improvements in patient-reported outcomes, particularly in reducing Pelvic Floor Distress Inventory scores. Overall, RASC was associated with longer operative times but lower blood loss, while Uphold™ had better anterior vaginal wall outcomes. Despite these differences, both procedures had low complication rates and demonstrated effectiveness in treating apical prolapse.
