This article presents the ESGE/ESGO/SERGS consensus on surgical steps for minimally invasive paraaortic lymphadenectomy in gynecologic oncology, covering both transperitoneal and extraperitoneal approaches. Through a Delphi process with experts from these three societies, 29 surgical steps for the transperitoneal approach and 17 for the extraperitoneal approach were defined, resulting in a standardized protocol. An Objective Structured Assessment of Technical Skills (OSATS) tool was also developed to assess and enhance the technical skills necessary for paraaortic lymphadenectomy. This standardized guideline aims to improve training, surgical consistency, and patient outcomes in gynecologic oncology.
