The article presents the ESGE/ESGO/SERGS consensus statement on the surgical steps for minimally invasive paraaortic lymphadenectomy in gynecologic oncology, focusing on both transperitoneal and extraperitoneal approaches. A working group of experts from the three societies collaborated to create a standardized description of the procedure and developed a specific Objective Structured Assessment of Technical Skills (OSATS) scale for each approach. Through a two-round Delphi process involving 19 experts, consensus was reached on 29 surgical steps for the transperitoneal approach and 17 steps for the extraperitoneal approach. The OSATS tool was designed to assess and standardize the performance of paraaortic lymphadenectomy, facilitating training and competency assessment in gynecologic oncology. This consensus provides a structured approach to ensure surgical excellence and improve outcomes in minimally invasive surgery.
