This study compares the outcomes of laparotomic radical hysterectomy (LRH) and minimally invasive radical hysterectomy (MISRH) in patients with early-stage cervical cancer. A retrospective review of 760 patients from 2002 to 2017 at Chang Gung Memorial Hospital analyzed surgical safety, overall survival (OS), disease-free survival (DFS), and recurrence. After propensity score matching (PSM), no significant differences were observed in 5-year OS or DFS between MISRH and LRH groups. However, MISRH resulted in a significantly shorter operation time, lower blood loss, and quicker recovery, but also a higher incidence of intraoperative bladder injury. The study concludes that MISRH is associated with comparable survival outcomes but a higher risk of urological complications compared to LRH.
