Prevalence and size of pelvic sentinel lymph node metastases in endometrial cancer

This study investigates the prevalence and size of pelvic sentinel lymph node (SLN) metastases in women with endometrial cancer, examining how these factors correlate with different histologic subtypes and risk factors such as myometrial invasion. Conducted at a tertiary academic center in Sweden, the research involved over 1,100 women who underwent robotic surgery with SLN detection using Indocyanine Green (ICG) as a tracer. The results showed that SLN metastases were present in 16.6% of the women, with a higher prevalence in those with high-grade and non-endometrioid cancers. Notably, isolated tumor cells (ITCs) were more commonly found in low-grade cancers, while larger metastases were more frequent in high-risk groups. The findings support the routine use of SLN procedures in all women with endometrial cancer, regardless of preoperative risk assessments, to ensure accurate staging and optimal treatment planning.
